This provides a score out of 100, with 35 points allocated to subjective findings and 65 points allocated to objective findings
The parameters assessed are:
• Pain (range of 0 to 15)
• Patient assesses their worst pain
• i. No pain -15pts
• ii. Mild pain -10pts
• iii. Moderate pain -5pts
• iv. Severe pain -0pts
• Activities of daily living (0-20pts)
• Activity level
• i. Full work -4pts
• ii. Full recreation -4pts
• iii. Unaffected sleep -4pts
• Positioning
• i. Up to waist -2pts
• ii. Up to xiphoid -4pts
• iii. Up to neck -6pts
• iv. Up to top of head -8pts
• v. Above head -10pts
• Range of motion (40 pts)
• Elevation and abduction (each 10pts)
• i. 0-30 0
• ii. 31-60 2
• iii. 61-90 4
• iv. 91-120 6
• v. 121-150 8
• vi. 151-180 10
• External rotation (10pts)
• i. Hand behind head with elbow held forward 2
• ii. Hand behind head with elbow held back 2
• iii. Hand on top of head with elbow forward 2
• iv. Hand on top of head with elbow back 2
• v. Full elevation from on top of head 2
• Internal rotation
• i. Dorsum of hand to lateral thigh 0
• ii. Dorsum of hand to buttock 2
• iii. Dorsum of hand to LS junction 4
• iv. Dorsum of hand to waist (L3) 6
• v. Dorsum of hand to T12 9
• vi. Dorsum of hand to T7 10
• Power
• Measure power of abduction at 90 degrees in pounds
• i. Subtract 1 for each pound below 25

Constant found on average a 3% interobserver variability when assessed with three examiners.
